GE 6W Standard Cool White
Found this lamp at a Restore inside a newer GE F6T5 sleeve. It was only a dollar and it looked like it still worked so I bought it. Looks like it's from the 50s or 60s due to the lack of ordering code and the standard cool white name instead of the the later cool white.

Thanks! That's my guess as well, I found it inside a modern GE F6T5 sleeve priced at $1, took a look inside and saw the lamp was old so I decided to take a chance on it and luckily it still worked.
